New Patient Registration

In accordance with our policy on diversity and equality, we welcome patients from all diverse groups.  

Patients must be a resident (or be intending to live) in Northern Ireland for longer than six months to register as a permanent patient with a Northern Ireland GP Practice.  

Patients from the Republic of Ireland who work in Northern Ireland (Cross Border Workers) also have the right to register with a Northern Ireland GP Practice.  This does not apply to family members of patients who fulfil this criteria. 

YOU WILL NEED TO DOWNLOAD & COMPLETE BOTH THE ARDMORE REGISTRATION FORM AND ONE OF THE BSO FORMS.

Patients wishing to join the practice who reside in Northern Ireland must provide the following documentation:

If you have a UK medical card please bring this to our reception. 

If you do not have a medical card but are currently registered with another UK GP practice you will be asked to complete a registration form.

If you are not a UK citizen and have moved to Northern Ireland you must complete a registration form and provide a copy of your passport (or an alternative form of photo ID) and any current visa you may have.

If you have moved from the Republic of Ireland (even if you are a UK citizen) you must also provide proof of your permanent address in Northern Ireland.

If you are a UK citizen but have been living outside of the UK you must complete a regsitration and provide a copy of your UK passport (or an alternative form of photo ID) and any current visa you may have.

In some cases a patient may be asked to provide additional information regarding themselves to enable the Business Service Organisation (BSO) to process their registration.  We will contact you if this is required by the BSO.

Once we have received all of your required and completed documentation we will make an appointment for you to see the practice nurse for a New Patient Medical Examination.  Please bring with you to your appointment a list of any regular medications you may be taking.
